Guidelines

1. Have been diagnosed with a terminal illness and have been given a life expectancy of six months or less if the disease follows its natural course.
2. Have a physician who agrees to Hospice care for the patient.
3. Be assessed by the Hospice Team for needs and desire of Hospice services.
